To purchase a Brainwise Training Supports Program using your NDIS funding for Capacity Building - Improved Daily Living Skills, simply provide your consent and the necessary details below. If you are plan-managed, we will send the invoice directly to your NDIS plan manager. For self-managed participants, the invoice will be emailed to your nominated address. Once we have received the consent form, you will receive an activation email. Our parenting program also includes non face to face appointments with Dr Shields to offer an individualised approach as needed.
